Resume

Objective: To evaluate ultrasound and Doppler markers in pregnant women with gestational diabetes (GDM) depending on insulin therapy. Materials and Methods: A retrospective cohort study was conducted on 600 pregnant women: 300 with GDM and 300 with physiological pregnancy. The GDM group was divided into diet therapy (n=180) and insulin therapy (n=120). Polyhydramnios, uteroplacental blood flow (UPBF) disorders, macrosomia, diabetic fetopathy, and neonatal transfers to the intensive care unit (NICU) were assessed. Statistics: Mann–Whitney U test, Pearson χ², Fisher’s exact test. Results: GDM was associated with increased incidence of polyhydramnios (7.0% vs 1.3% in controls; p=0.0013) and UPBF disorders (27.7% vs 20.7%; p=0.045). The mean gestational age at insulin therapy initiation was 32.27 ± 2.67 weeks. Delayed initiation of insulin therapy increased the incidence of diabetic fetopathy (8.3% vs 2.2% in diet therapy, p=0.0136) and NICU transfers (12.5% vs 5.6%, p=0.028), with no significant differences in macrosomia (14.2% vs 13.9%, p=0.877). Conclusion: Early ultrasound and Doppler changes occur before macrosomia; markers of fetopathy and polyhydramnios require immediate insulin therapy.
