Ovarian endometrioma is one of the most common forms of genital endometriosis and is characterized by pronounced morphological and stromal-vascular changes affecting the functional state of the organ. This study analyzes the morphological and immunohistochemical features of ovarian tissue in patients aged 18–22 years. The material consisted of surgical ovarian tissue samples obtained after removal from patients with endometrioma. Morphological examination was performed using Van Gieson staining, while immunohistochemical analysis was carried out using desmin and vimentin markers with DAB chromogen visualization. It was established that endometrioma is characterized by the presence of cystic endometrioid glands, pronounced sclerotic changes in the stroma, disorganization of collagen fibers, thickening of vascular walls, and narrowing of their lumen. Immunohistochemical analysis revealed increased vimentin expression, as well as the presence of desmin-positive cells in the vascular wall and stroma, reflecting activation of stromal elements and tissue remodeling processes. These findings indicate the development of a chronic inflammatory process, fibrosis, and disruption of the structural organization of ovarian tissue in endometriosis.
