MORPHOLOGICAL FEATURES OF CONGENITAL SPINAL HERNIAS IN CHILDREN AND THEIR SIGNIFICANCE FOR THE CHOICE OF TREATMENT STRATEGY

Resume

A clinical and morphological study of congenital spinal hernias in children was carried out based on the analysis of 39 biopsy specimens of the hernial sac and regional tissues obtained from patients under 1 year of age. In 21 children, the material was obtained during surgical treatment of meningoradiculocele, while in 18 cases spinal hernias were combined with congenital intradural malformations. It was established that the morphological structure of the hernial contents is characterized by pronounced polymorphism. In cases with intact skin, no specific differences in the epidermis were found between various forms of spina bifida; however, in the presence of ulceration, marked vascular-trophic disorders, dermal edema, vascular thrombosis, and granulation tissue were observed. Fragments of the meninges, nerve roots, neuroglia, mature adipose tissue, cystic cavities, as well as associated malformations such as lipomas, angiolipomas, fibromas, osteophytes, arachnoid cysts, and inter-root adhesions were identified within the hernial sac. The morphological findings confirm that in complicated forms of the defect, the radicular apparatus and elements of the spinal cord conducting system are frequently involved, which determines the severity of neurological disorders and requires a precise surgical approach.
