Type 2 diabetes mellitus is one of the most common chronic noncommunicable diseases and is characterized by a high risk of complications, reduced quality of life, and a significant medical and social burden. Depressive disorders occupy a special place among comorbid conditions in type 2 diabetes, occurring significantly more frequently in this category of patients than in the general population and having a significant impact on the course of the disease, glycemic control, treatment adherence, and prognosis. This article presents a review of current data on depressive disorders in type 2 diabetes, examining the bidirectional relationships between diabetes and depression, the clinical significance of affective symptoms in diabetology, the challenges of differential diagnosis, and current approaches to psychosocial screening and clinical verification of depression. It demonstrates that depression in patients with type 2 diabetes should not be considered a secondary emotional reaction to a chronic disease, but rather an independent, clinically significant factor requiring active identification and inclusion in a comprehensive, multidisciplinary patient management system.
