MODERN APPROACHES TO LEG AMPUTATION FOR DIABETIC GANGRENE OF THE LOWER EXTREMITIES

Resume

Objective: Improving of the results of amputation at the leg level with the study of angioarchitectonics of the leg in patients with diabetic gangrene of the lower extremities. Material and methods: the results of the examination and treatment of 47 patients with diabetic gangrene of the lower extremities who underwent amputation at the level of the upper third of the leg analyzed. Results: Analysis of the results of the patients showed that in 40 (85.1%) patients amputation stumps healed by primary intention. Postoperative suppuration of the lower leg stump was observed in 4 (8.5%), in all patients the wounds healed by secondary intention. In 2 (4.3%) patients, signs of ischemia of the stump were observed in the postoperative period and, at the same time, were limited to fluffing of the stitches without necrectomy, and 1 (2.1%) patients needed reamputation. There were not lethal outcomes. Conclusions: Amputation at the level of the lower leg, taking into account the structure of the arterial blood supply to local tissues, makes it possible to primary stump healing in 85.1% of cases. Anatomically justified resection of the bones of the leg provides a more rapid emergence of primary bone callus in the stump area and a reduction in the duration of inpatient treatment.
