Resume

The recurrence of ulcerative and erosive gastroduodenal bleeding in patients with a pronounced comorbid background remains a major challenge in emergency surgery. Traditional prognostic systems do not fully reflect hidden pathogenetic disorders, particularly systemic and local endothelial dysfunction. A comprehensive analysis of the treatment outcomes of 338 patients with non-variceal bleeding revealed a high level of polymorbidity (mean Charlson index of 4.8±1.8 points) and a predominance of diffuse erosive mucosal lesions covering multiple anatomical zones. The implementation of a risk-oriented prognostic system integrating clinical and endoscopic parameters with markers of endothelial activity (endothelin-1, nitric oxide, VEGF) allows for the objective identification of high-risk groups. This approach provides a differentiated selection of endoscopic hemostasis tactics and early endovascular interventions, contributing to the effective personalized prevention of recurrent hemorrhages in comorbid patients.
