Resume
Objective: To develop and evaluate the clinical effectiveness of a three-stage rehabilitation program based on McKenzie MDT with integration of stabilization and neurodynamic training after lumbar decompression surgery. Methods: Controlled study of 187 patients randomized into three groups: McKenzie MDT + basic therapy (n=63), standard therapeutic exercise + basic therapy (n=62), and basic therapy only (n=62). Outcomes assessed at T0, T1 and T2 (6 months). Results: At T2, the MDT group demonstrated VAS reduction of 66.0% (vs. 48.1% and 33.3%), ODI decrease of 27.4 pp (reaching "minimal disability" category), Schober's test recovery to normal (6.1±0.6 cm), EMG tibialis anterior amplitude increase of 18.0%, and 28.2% reduction in CoP velocity in eyes-closed posturography. All between-group differences were statistically significant (p<0.05). Conclusion: The three-stage McKenzie program is superior to standard approaches across clinical, neurophysiological and posturological parameters, with a continuing effect at follow-up attributable to the educational component.
