BOLALARDA ALLERGIK RINOSINUSITNING TERI ALLERGOSINOVLARI MANFIY BO‘LGAN HOLATLARIDA IGE, IL-33 VA TSLP NING DIAGNOSTIK ROLI

Rezyume

Dolzarblik. Bolalarda uchraydigan mavsumiy allergik rinosinusit (MAR) ko‘pincha teri allergosinvlarining manfiy natijalari bilan kechadi, bu esa tashxis qo‘yishni va davolash taktikasini tanlashni qiyinlashtiradi. Tadqiqot maqsadi — teri allergoprobalarining manfiy natijalari fonida MAR bilan og‘rigan bolalarda burun shilliq qavati darajasida lokal va tug‘ma sezuvchanlikning (sensibilizatsiyaning) uchrash chastotasini hamda ularning klinik-laborator xususiyatlarini baholash. Materiallar va usullar. 2022–2025 yillar oralig‘ida o‘tkazilgan prospektiv tadqiqotga 7–14 yoshdagi 96 nafar bola kiritildi. Bemorlar uch guruhga bo‘lindi: klassik allergik rinit (AR) (n = 40), yashirin (lokal) AR (n = 38) va nazorat guruhi (n = 18). Tashxis algoritmiga endoskopiya, teri prick-testlari, zardob va burun sekretsiyasida spesifik IgE ni aniqlash, shuningdek, burun sekretsiyasida IL 33 va TSLP darajalarini aniqlash (IFA usuli) kiritildi. Natijalar. Teri allergosinvlarining manfiy natijalariga ega bolalar orasida 67,9% holatda burun sekretsiyasida spesifik IgE mavjudligi yoki IL 33 va TSLP darajalarining oshishi ko‘rinishida namoyon bo‘ladigan lokal sezuvchanlik yoki tug‘ma allergik yallig‘lanish aniqlangan. Yashirin AR guruhi bolalarida epitelial alarminalar (IL 33 va TSLP) darajasi klassik AR guruhiga nisbatan sezilarli darajada yuqori bo‘ldi (p < 0,001). Intranasal glyukokortikosteroidlar bilan davolash har ikki guruhda ham yuqori samaradorlikni ko‘rsatdi (TNSS ko‘rsatkichining 70–75% gacha kamayishi). Xulosa. Burun sekretsiyasida spesifik IgE va epitelial alarminalar (IL 33, TSLP) ni aniqlashni diagnostik algoritmga kiritish teri allergoprobalarining manfiy natijalari fonida ham yashirin allergik shakllarni aniqlash va «noallergik» rinit tashxisi qo‘yilgan bolalarda patogenetik asoslangan terapiyani belgilash imkonini beradi.
