REPRODUKTIV YOSHDAGI BEMORLARDA BACHADON BO‘YNI PAPILLOMA VIRUSI (HPV) BILAN BOG‘LIQ ZARARLANISHLARINING DIAGNOSTIKASINI OPTIMALLASHTIRISH

Bachadon bo‘yni saratoni ayollar reproduktiv salomatligiga jiddiy xavf tug‘diruvchi onkoginekologik kasalliklardan biri bo‘lib, uning rivojlanishida yuqori onkogen xavfga ega odam papilloma virusi (Human papillomavirus — HPV) bilan persistirlovchi infeksiya yetakchi etiologik omil hisoblanadi. HPV infeksiyasini o‘z vaqtida aniqlash, virusning yuqori onkogen genotiplarini differensial baholash hamda bachadon bo‘yni epiteliysidagi erta o‘zgarishlarni aniqlash servikal intraepitelial neoplaziya va invaziv saraton rivojlanishining oldini olish imkonini beradi. Maqsad. Reproduktiv yoshdagi ayollarda HPV bilan bog‘liq bachadon bo‘yni zararlanishlarini erta aniqlash samaradorligini oshirish uchun molekulyar-genetik, sitologik va kolposkopik tekshiruvlarni kompleks qo‘llashga asoslangan diagnostik algoritmni takomillashtirish. Material va usullar. Bachadon bo‘yni patologiyasi yoki HPV infeksiyasiga shubha qilingan reproduktiv yoshdagi ayollarni tekshirishda klinik-anamnestik baholash, ginekologik ko‘rik, HPV-DNA testi, yuqori onkogen xavfli HPV genotiplarini aniqlash, suyuqlik asosidagi yoki an’anaviy sitologik tekshiruv, kengaytirilgan kolposkopiya hamda ko‘rsatmalarga muvofiq bachadon bo‘yni biopsiyasi va gistologik tekshiruvlardan foydalanishga asoslangan kompleks yondashuv tahlil qilindi. Natijalar. HPV-DNA testini birlamchi molekulyar skrining usuli sifatida qo‘llash va HPV-musbat bemorlarni genotiplash, sitologik va kolposkopik ko‘rsatkichlar asosida xavf guruhlariga ajratish bachadon bo‘yni epitelial o‘zgarishlarini erta aniqlash imkoniyatini oshiradi. Ayniqsa, HPV 16 va HPV 18 aniqlangan, virus persistensiyasi kuzatilgan hamda sitologik va kolposkopik o‘zgarishlari mavjud bemorlarda chuqurlashtirilgan tekshiruv zarur. Xulosa. Reproduktiv yoshdagi ayollarda HPV bilan bog‘liq bachadon bo‘yni zararlanishlarini diagnostika qilishda HPV-DNA testi, genotiplash, sitologiya va kengaytirilgan kolposkopiyani yagona xavfga asoslangan algoritmga birlashtirish diagnostikaning klinik samaradorligini oshiradi va yuqori darajali servikal intraepitelial neoplaziyalarni o‘z vaqtida aniqlashga yordam beradi.
