The article presents a standardized 12-month monitoring protocol for noncavitated and early carious lesions in children with diabetic polyneuropathy. The prospective controlled cohort includes 130 children aged 7-15 years: 85 children with dental caries in the setting of diabetic polyneuropathy and 45 healthy age-comparable controls. The clinical cohort is divided into a conventional preventive subgroup (n=40) and a multidisciplinary preventive subgroup (n=45). Baseline assessment combines DMFT/DMFS, OHI-S, ICDAS II, 655-nm laser fluorescence, 2% methylene-blue indication and a structured saliva panel. Short-term adherence and lesion indication are reviewed after the preventive course and at three months, while caries experience, lesion stage and selected salivary variables are reassessed at six and twelve months. The protocol emphasizes identical examination conditions, lesion-level documentation and cautious interpretation of change in a non-randomized real-world cohort.
