Resume
The aim of this review was to systematize current evidence on risk factors, opportunities for early detection, patient referral, and interdisciplinary coordination in neoplastic diseases of the ENT organs and adjacent anatomical regions of the head and neck, as well as to provide a separate assessment of the current situation in the clinical practice of Uzbekistan, the Commonwealth of Independent States (CIS), and worldwide. The review included 40 sources published between 2005 and 2025, comprising international epidemiological reports, systematic reviews and meta-analyses, clinical guidelines, original studies, and regional publications. Global evidence indicates that there is currently insufficient evidence to support universal population-based screening for all tumors of the ENT organs. At the same time, risk-oriented active case finding has considerable practical value in patients exposed to tobacco and alcohol, individuals with potentially malignant mucosal lesions, persistent dysphonia, dysphagia, cervical lymphadenopathy, and other warning signs suggestive of malignancy. For Uzbekistan, particular importance is attached to the limited availability of direct population-based data, regional differences in epidemiological indicators, and the need to establish a unified referral pathway linking primary health care, dental services, otorhinolaryngologists, and oncologists. In the CIS countries, characteristic problems include delayed morphological verification, territorial disparities in access to diagnostic services, and a high dependence of diagnostic outcomes on the quality of the initial clinical examination. A standardized interdisciplinary algorithm for oncological alertness appears to be the most promising approach. Such an algorithm should integrate initial risk assessment, endoscopic examination, needle or incisional biopsy, and, where clinically indicated, molecular marker testing within a unified time-based and organizational pathway.
