TREATMENT TACTICS OF PURULENT-NECROTIC LESIONS OF THE LOWER EXTREMITIES IN DIABETES MELLITUS

Resume

The number of patients with diabetes mellitus (DM) in Andijan region, which were registered according to endocrinologists from 2000 to 2015 has almost quadrupled from 26000 to more than 110000 in 2016. If we consider that 15-20 years after the onset of the disease in patients with diabetes develop various pyo-necrotic lesions of the lower extremity (PNLLE), it shows that the number of patients with diabetic foot syndrome (DFS) in diabetes will also increase. According to the International Diabetes Federation (IDF) WHO, the number of patients increases by 5-7% annually. Special calculations of IDF experts show that by 2030 the number of diabetes patients in the world will increase 1.5 times and will reach 450 million people and equals that every 15 people will have diabetes, and in 2050 this figure will be equal to every 6 people. According to the World Health Organization (WHO) Committee of Experts on Diabetes Mellitus, “Diabetes and its vascular complications will be an ever-increasing public health burden.

The aim of the work was to determine the ways to improve the results of critical ischemia DMF treatment using etiopathogenetically similar drugs.

The objectives of the study are

– Studying the deep pathogenetic basis of the development of SDS and the development of purulent-necrotic lesions in DM;

– Creation of a method to influence the pathogenetic basis of DM development;

– Development and implementation of a new intra-arterial method of exposure to etiopathogenetically selected drugs in the practice of public health.In order to perform the set tasks of the patients with purulent-necrotic complications of SDS, which constitute the main part (75-80%) of the patients of the department of surgical infection of the ASMI clinic. We analyzed the results of treatment of 230 patients with PNLLE who developed purulent-necrotic process. Study of deep pathogenetic basis of development of SDS and development of purulent-necrotic lesions in DM allows to create the way of influence on pathogenetic basis of development of SDS as well as to develop and implement the proposed complex of drug treatment intraarterial method of etiopathogenetically selected drugs into the practice of public health.
