DIFFERENTIATED CRURORRHAPHY FOR HIATAL HERNIA: QUALITY OF LIFE AND OUTCOME PREDICTORS

Resume

Objective: to evaluate quality of life after laparoscopic crurorrhaphy with fundoplication for hiatal hernia (HH) with defect >3.5 cm — with and without Dualmesh augmentation — and to identify outcome predictors. Materials and Methods. Retrospective analysis of 79 patients (2019–2023). Group I (n=45): crurorrhaphy with Dualmesh. Group II (n=34): suture crurorrhaphy. Assessment: GERD-HRQL, Visick, 24-month follow-up. Results. Median GERD-HRQL decreased from 18.0 to 2.0 (89%; p<0.001). Visick I–II — 83.5%. Clavien–Dindo I–II complications — 10.1%; III–V — 0%. Radiological recurrence: 6.7% vs 11.8% (p=0.46); symptomatic — 3.8%. Predictors of Visick III–IV: GERD-HRQL >25 (OR 3.2; p=0.032), BMI >30 (OR 2.8; p=0.048); EPV=2.6. Conclusion. Laparoscopic crurorrhaphy with fundoplication provides sustained quality-of-life improvement. The Dualmesh algorithm shows a trend toward reduced recurrence.
