Relevance of the topic. Mild closed traumatic brain injury (MCTBI) occupies a leading place in the structure of neurotrauma. Tens of millions of cases of traumatic brain injury are registered annually worldwide, with up to 70–80% of these cases being mild. The phenotypic structure of pain syndrome in the late period remains unresolved, which complicates the development of differentiated diagnostic and therapeutic approaches. The aim of the study was to identify the features of the clinical structure of pain syndrome, assess its intensity in the formation and maintenance of post-traumatic headache. Material and methods of the study. The work included 164 patients of young (18-44 years) and middle aged (44-59 years), divided into two groups comparable in age structure: patients with headache after LZTBI (83 people) and patients with primary forms of headache without a history of trauma (81 people). Results of the study. In the main group (MG), persistent post-traumatic headache prevailed and was detected in 72 (88.9%) patients, and in the CG PTHA was not registered. Migraine was diagnosed in 28 (34.6%) patients in the MG, in 14 people (16.9%) in the CG. The migraine phenotype was twice as common in patients after injury. In the CG, this variant was recorded more frequently (38.6%) compared to the MG (11.1%). Venous headache was more frequently detected in the MG – 27.2% versus 9.6% in the CG. Conclusion. The obtained data indicate the polymorphism of post-traumatic headache and its association with primary mechanisms.
