Recent years have been characterized by the rapid advancement of innovative medical technologies, resulting in substantial progress in the development of therapeutic strategies for purulent soft tissue diseases. Contemporary approaches increasingly incorporate a wide spectrum of physical wound decontamination techniques, including laser-assisted technologies, vacuum therapy, radiosurgical interventions, hydropressive systems, pulsatile antiseptic irrigation, ultrasonic cavitation, photodynamic therapy, the application of exogenous nitric oxide, and other modern therapeutic modalities. The objective of the present study was to enhance the clinical outcomes in patients suffering from purulent surgical diseases of the soft tissues of the upper and lower extremities through the implementation of wound hyperventilation using directed oxygen flow. Materials and Methods: The study involved a comprehensive analysis of diagnostic and therapeutic data obtained from 122 patients presenting with purulent diseases of the extremities. Of these, 68 patients were allocated to Group I (control group), in which conventional therapeutic protocols were employed. Group II (the principal study group) comprised 54 patients who, in addition to standard treatment, underwent wound hyperventilation with a continuous oxygen flow. Results: All patients diagnosed with purulent soft tissue infections underwent emergency surgical intervention on the day of admission, including incision and drainage of the purulent focus followed by antiseptic debridement of the infected cavity using a 3% hydrogen peroxide solution. Subsequently, wound hyperventilation with oxygen streams was performed, followed by the application of Levomekol ointment under sterile gauze dressings twice daily. Therapeutic management further incorporated the fundamental principles of purulent wound care alongside systemic antibiotic therapy administered in accordance with the antimicrobial sensitivity profile of the wound microflora. More than one-third of the surgical procedures were conducted under local anesthesia. It should be emphasized that the subsequent therapeutic management of patients with postoperative purulent wounds and those with purulent soft tissue diseases was identical in both study groups. Conclusion: The implementation of the authors’ newly developed technique of wound hyperventilation with directed oxygen flow as part of the comprehensive treatment of patients with purulent soft tissue diseases of the extremities demonstrated significant clinical efficacy. The principal indicators reflecting the dynamics of the wound-healing process showed accelerated improvement, including early wound decontamination from infection as early as the 2nd day of treatment, the onset of granulation tissue formation by the 4th day, and the initiation of epithelialization by the 6th day of therapy. These findings substantiate the therapeutic effectiveness and clinical feasibility of the proposed treatment modality.
