This study is aimed at evaluating the clinical and diagnostic significance of estrogen metabolite imbalance in women with adenomyosis and impaired reproductive function. The study included 32 reproductive-aged women with adenomyosis and infertility, as well as 30 healthy women who formed the control group. All participants underwent comprehensive examination, including clinical, ultrasound, biochemical, and endoscopic methods. Urinary concentrations of estrogen metabolites (2-hydroxyestrone, 4-hydroxyestrone, and 16α-hydroxyestrone) were determined using the ESTRAMET 2/16 ELISA test system. The results demonstrated a significant increase in proliferative estrogen metabolites, particularly 16α-hydroxyestrone and 4-hydroxyestrone, along with a decrease in the protective metabolite 2-hydroxyestrone in patients with adenomyosis (p<0.01). A reduced 2-hydroxyestrone/16α-hydroxyestrone ratio was associated with clinical signs of hormonal imbalance, anovulation, and infertility. These findings confirm the high diagnostic value of estrogen metabolite profiling and highlight its potential for developing personalized hormonal correction strategies in women with adenomyosis.
