DIAGNOSIS AND SURGICAL MANAGEMENT OF PERSISTENT CLOACA IN GIRLS

Resume

Objective. To identify effective approaches for improving diagnostic accuracy and increasing the effectiveness of surgical treatment of persistent cloaca in girls. Materials and Methods. This study included 33 female patients diagnosed with persistent cloaca who were observed and treated at the National Children’s Medical Center between 2021 and 2025. All patients underwent comprehensive diagnostic evaluation, including ultrasonography, magnetic resonance imaging, contrast cloacoscopy, and distal colostography. Diagnostic laparoscopy was performed when indicated to clarify pelvic anatomy and associated anomalies. The anatomical variants of the malformation, length of the common channel, associated congenital anomalies, applied surgical techniques, and postoperative complications were analyzed. Results. In 43.3% of patients, the length of the common channel was up to 2.0 cm; in 50.0%, it ranged from 2.1 to 3.5 cm; and in 6.7%, it exceeded 4.0 cm. A posterior sagittal approach with single-stage anorecto-urethro-vaginoplasty was performed in 83.3% of cases. In patients with a long common channel, laparoscopic or laparotomic approaches were required. Postoperative complications included colostomy prolapse, secondary wound healing, fecal incontinence, and combined fecal and urinary incontinence. Most patients demonstrated satisfactory short- and mid-term functional outcomes during follow-up exceeding four years. Conclusions. The length of the common channel and anatomical features of persistent cloaca play a decisive role in selecting surgical strategy and predicting functional outcomes. Comprehensive diagnostics, timely colostomy formation, and staged reconstructive surgery in specialized centers significantly improve treatment results.
