MODERN SURGICAL STRATEGY FOR THE TREATMENT OF COMBINED SPINAL AND PULMONARY TUBERCULOSIS

The study was based on the analysis of 86 patients with spinal tuberculosis combined with pulmonary tuberculosis who received treatment in the Department of Surgical Treatment of Osteoarticular Tuberculosis at the clinic of the Republican Specialized Scientific and Practical Medical Center of Phthisiology and Pulmonology of the Ministry of Health of the Republic of Uzbekistan (RSSPMCP&P) during 2015–2024. The control group consisted of 54 patients with spinal tuberculosis without pulmonary tuberculosis involvement. Combined spinal and pulmonary tuberculosis is characterized by a severe clinical course with pronounced dysfunction of vital organs and systems. Extensive spinal involvement with the formation of paravertebral abscesses was detected in 89.3% of patients, while spinal cord disorders of varying severity were diagnosed in 67.4% of cases. Active pulmonary tuberculosis was identified in 77.9 ± 4.2% of patients with spinal tuberculosis; pulmonary tissue destruction was observed in 31.4% of cases, and bacterial excretion (MBT+) was registered in 33.7% of patients, indicating a high activity of the specific pathological process. Comprehensive treatment, including rational antibacterial therapy, pathogenetic management, and early surgical intervention for spinal tuberculosis combined with pulmonary tuberculosis, ensured clinical cure or stable stabilization of the specific process in 69.5% of cases within one year after the inpatient stage of treatment.
