FEATURES OF THE DEVELOPMENT OF SYMPTOMATIC EPILEPSY IN CLINICAL FORMS OF CHILDHOOD CEREBRAL PALSY

Cerebral palsy (CP) and epilepsy are among the most disabling diseases of pediatric neurology. The clinical course of cerebral palsy becomes more complicated when it is complicated by symptomatic epilepsy (SE). The purpose of the study was to study the course of SE in various forms of cerebral palsy. In 2020-2023, 95 children with cerebral palsy of various clinical forms were examined at the Republican Children’s Psychoneurological Hospital. Children are divided into 3 groups depending on their physiological characteristics: 3-6, 7-11, 12-14 years. Depending on the presence and absence of SE in cerebral palsy, children were divided into 2 groups: 1) main group – 45 children with cerebral palsy with epileptic seizures, 2) comparison group – 50 children without SE in cerebral palsy. In our studies, the most common form of cerebral palsy was spastic diplegia – in 34 (35.4%) children, hemiplegia – in 24 (25%), double hemiplegia – in 18 (19.1%), atonic-astatic – in 10 (10.7%), the hyperkinetic form was observed in 9 (9.7%) children. The disease was most common in children 1-3 years old, and least common in children 12-14 years old. Epilepsy was observed in 45 (47.4%) children with cerebral palsy, it was not observed in 50 (52.6%) children. We investigated the relationship between the development of SE and the clinical form of cerebral palsy. SE was detected in 47.4% of children in hemiplegic and bilateral hemiplegic forms, depending on the clinical picture of cerebral palsy. The clinical form of SE is directly related to the clinical form of cerebral palsy. Generalized epileptic seizures were recorded with the maximum frequency of bilateral hemiplegia, spastic diplegia and hemiplegic form of cerebral palsy. These results make it possible to determine the onset and course of clinical manifestations of epileptic paroxysms, as well as additional neurodiagnostic methods. This makes it possible to choose the strategy and tactics of anticonvulsant therapy.
