Resume

The purpose of the study. Optimizing Perioperative Analgesia Using a Multimodal Approach with Intravenous Ibuprofen in Combination with Paracetamol in Pediatric Abdominal Surgery. Research material. 105 patients (aged 5-17 years) with abdominal surgical pathology treated at the Department of Planned Surgery and RIT of the National Children's Medical Center were studied: cysts of the choledox (common bile duct), pancreas, spleen, liver echinococcosis, choledocholithiasis, cholecystitis, Meckel's diverticulum, benign large tumors of the abdominal cavity, abdominal cavity. Research period - 2020-2023. Patients were divided into two groups: group 1 (main n = 75), combined MM analgesia was performed according to the scheme: prophylactic administration of paracetamol at a dose of 15 mg/kg t/i 15 minutes before the operation, then Ibuprofen 15 minutes before the end of the operation Administer t/i at a dose of 10 mg/kg. Repeated anesthesia with Ibuprofen at a dose of 10 mg/kg was performed after 5-6 hours. On the 2nd day, Ibuprofen at a dose of 10 mg/kg was administered 1-2 times on average. Group 2 (comparison, n = 30, where traditional systemic opioid analgesia was performed with intramuscular morphine at a dose of 0.2-0.3 mg/kg. On the first day, morphine was injected 3 times, on the 2nd day - 2 times a day Perioperative cardiomonitoring was performed, the dynamics of CRO, IL-6 and glucose levels were studied, and the intensity of POS was evaluated on a visual-analog scale and the scale of satisfaction with analgesia at the main stages of the study. Results: In children, multimodal analgesia in combination with Paracetamol at the above doses and in operations increases the level of nociceptive protection and provides high efficiency of postoperative analgesia without serious side effects. The reliable dynamics of the CRO level reflects a clinically significant anti-inflammatory effect and a clear decrease in pain impulses in patients of the main group..

Keywords: surgical stress, multimodal analgesia, analgesic effect.
