Resume
Laparoscopic cholecystectomy (LCE) in combination with endoscopic retrograde cholangiopancreatography, papillosphincterotomy and lithoextraction (LCE+ERCP) or laparoscopic choledocholithotomy (LCE+LHLT) are the most common single-stage combined surgical methods for the treatment of cholecystocholedocholithiasis (XXL). A retrospective analysis of the results of treatment of patients up to 2018 (control group) and from 2018 (main group), using the ASA-PS, Charlson Comorbidity Index (CCI) scales), as well as the classification of the severity of chronic jaundice (E.I. Halperin from 2012, hereinafter the Halperin index) for a comprehensive assessment of the severity of the condition of patients. The total length of hospital stay in the main group decreased, in patients who underwent LHE+ERCP by 4.0± 1.0 bed days, in patients who underwent LHE+LHLT by 3.5± 1.2 bed days. The total number of thromboembolic complications in the main group was 4.1%, which is less than in the control group (16.6%). Stroke and myocardial infarction were suffered by patients from the control group in 1 case, which amounted to 5.5% and 5.5%, respectively. In the control group, pneumonia was noted in 2 (11.1%) patients, in the main group in 1 (4.1%) patient. The mortality rate in the control group was 5.5%, and there were no deaths in the main group.
Key words: Choledocholithiasis, mechanical jaundice; laparoscopic cholecystectomy, laparoscopic choledocholithotomy, Halperin index, Charlson Comorbidity Index (CCI).
