Acute appendicitis, a common surgical emergency, presents specific difficulties when it occurs in the elderly. Although it accounts for 5-10% of appendicitis cases, the incidence rate in the elderly is approximately 1.5 to 2 cases per 10,000 person-years. This review explores the epidemiology, clinical presentation, diagnostic issues, treatment options, and outcomes of appendicitis in patients aged 65 and older. Elderly individuals often display atypical symptoms, resulting in diagnostic delays and a higher rate of complications. Perforation rates in this group range from 30-50%, much higher than the 20% seen in younger patients, and mortality rates are between 5-15%, compared to less than 1% in younger populations. The presence of comorbidities and atypical symptoms further complicates diagnosis. Imaging, particularly CT scans, is essential due to its high accuracy. Treatment plans must be individualized, balancing the risks and benefits of surgical versus non-surgical options. Laparoscopic appendectomy, favored for its minimally invasive nature, has shorter recovery times, though patient health must be carefully considered.
