Objective: study of the dynamics of the prevalence of HIV infection in the territory of the subjects of the Central Chernozem Region. Materials and methods: The main data source for the study was open statistical materials on HIV infection in the Central Federal District in the period from 2019 to 2023, a sample of subjects of the Russian Federation belonging to the Central Chernozem region was conducted. All the necessary information was taken from the website of the Federal State Statistics Service, as well as data from the Center for the Prevention and Control of AIDS, Central Research Institute, ITPC-ECCA, the results of scientific publications on this topic Study results: From 2019 to 2023, the incidence rate in the Central Chernozem region decreased by 28.2%. The highest HIV incidence rates were recorded in the Voronezh and Lipetsk regions (25.75 ±0.14 and 25.45 ±0.08); the lowest - in the Belgorod, Kursk, Tambov regions (13.54 ±0.12, 18.57 ±0.17 and 17.4 ±0.11). From 2019 to 2023, in the Central Chernozem Region, 68.69 ±0.1% of people contracted HIV through sexual transmission, and 31.31 ±0.21% through parenteral transmission. The proportion of men prevails over the proportion of women (64.82 ±0.04%, 35.18 ±0.07%). In terms of age structure, people aged 30-49 are most often infected (65.82 ±0.04%). The mortality rate is decreasing every year (from 1.94 ±0.1 to 1.71 ±0.18). The highest average mortality rate is in the Voronezh Region (2.26 ±0.02). In the period from 2019 to 2023, the number of tests for human immunodeficiency virus in the Central Chernozem Region increased by 26.1%. Also, from 2019 to 2022, 3167.95 ±0.13 people per 100,000 population received antiretroviral therapy in the Central Chernozem Region, with the Voronezh and Lipetsk Regions becoming the leaders in the use of ART (186.13 ±0.16 and 176.78 ±0.12). Conclusion: The results of the statistical analysis indicate the dynamics of a decrease in HIV cases in the Central Chernozem Region for the period 2019-2023. With a general trend towards a decrease in mortality from this disease, the situation in the Belgorod, Voronezh and Kursk Regions remains stable. Epidemiological data indicate that men in the 30-49 age group are most susceptible to infection with the human immunodeficiency virus, while the sexual route of infection is predominant over the parenteral route, however, in the Voronezh region from 2019 to 2021, the parenteral route of infection prevailed. In 2020, as a result of the COVID-19 pandemic and the introduction of restrictive measures, there was a decrease in the number of HIV tests performed, which was reflected in a decrease in the detection of new cases of the disease. At the same time, the number of HIV tests from 2019 to 2023 increased by 26.1%. Despite the ongoing state programs for ART and testing, the current epidemiological situation with HIV infection requires strengthening measures to educate the population about transmission routes and prevention.
