Resume

The article addresses modern approaches to the diagnosis of disturbances in the local defense system (LDS) of the respiratory tract in patients with chronic lung diseases (CLD). A multi-stage diagnostic algorithm is presented, encompassing the evaluation of anamnesis data, clinical and laboratory indicators, formulation of assumptions regarding the nature of LDS defects, and their confirmation through immunological and functional tests. It is noted that the application of this approach makes it possible to identify LDS impairments in 57.2% of CLD patients. The paper describes methods for correcting identified defects, including the use of levamisole to stimulate phagocytic activity and the T-cell immune component, intermittent plasmapheresis in steroid-dependent bronchial asthma, administration of pyrogenal and immunoglobulins in immunodeficiencies, as well as lysozyme inhalations to improve mucociliary clearance. The implementation of these comprehensive immunocorrective strategies contributes to reducing the frequency and severity of exacerbations, lowering the need for steroids and antibacterial agents, and achieving a stable therapeutic and preventive effect in patients with CLD.
