Resume

Neuropathic pain in patients with metastatic breast cancer remains one of the most pressing issues in palliative oncology, significantly reducing quality of life. Existing diagnostic tools (VAS, DN4, BPI, SF-36) have several limitations: insufficient sensitivity, inability to provide differential diagnosis, and a high cognitive burden for patients with asthenia. This study presents an innovative questionnaire, NEURO-MBC, developed for a comprehensive assessment of pain syndrome in oncology patients. The tool includes 20 items (16 patient-reported and 4 clinician-assessed), combining subjective self-assessment with objective clinical findings, while accounting for tumor-related and iatrogenic nerve damage. Validation (112 patients) demonstrated strong psychometric properties: Cronbach’s alpha = 0.89; sensitivity — 88%, specificity — 90%; AUC = 0.85. The use of NEURO-MBC increased the accuracy of diagnosing the neuropathic pain component by 32% and reduced the time required to select adequate analgesic therapy by 4 days. Integration of this tool into palliative oncology clinical practice ensures a personalized approach to pain management and improves standards of care for patients with metastatic breast cancer.
