Abnormal uterine bleeding (AUB) is one of the most common gynecological disorders among women of reproductive age. Endometrial dysfunction, characterized by impaired cyclic changes and vascularization of the endometrium, is considered a leading cause. Ultrasonographic parameters (endometrial thickness, ovarian volume, and antral follicle count) are important diagnostic markers, yet their value in differential diagnosis and prognosis remains underexplored. Objective. To evaluate ultrasonographic parameters in women with AUB associated with endometrial dysfunction and assess their utility in differential diagnosis and prognosis. Materials and Methods. A total of 120 women were examined between 2023 and 2025: 90 patients with AUB-E and 30 healthy controls. Participants were divided into three age groups: I (18–25 years), II (26–35 years), III (36–41 years). All underwent transvaginal ultrasonography to assess endometrial thickness, echotexture, ovarian volume, and AFC. Statistical analysis was performed using SPSS 26.0. Results. Functional changes (hyperechogenicity, mild heterogeneity, reduced AFC) predominated in groups I and II, while organic changes (endometrial atrophy, hyperplasia, stromal fibrosis) were more frequent in group III. Endometrial thickness and ovarian volume significantly correlated with clinical manifestations of bleeding (p<0.05). ROC analysis confirmed the predictive value of endometrial thickness (AUC=0.76) and ovarian volume (AUC=0.80) for recurrence risk. Conclusion. Comprehensive evaluation of ultrasonographic parameters is an effective tool for differential diagnosis and prognosis of AUB-E. Their integration into clinical practice may enhance individualized management and reduce recurrence risk.
