STUDY OF ACUTE KIDNEY INJURY IN CHILDREN WITH NEPHROTIC SYNDROME

Acute kidney injury (AKI) remains one of the most frequent and severe complications in children with nephrotic syndrome. Despite advances in diagnostics and therapy, the prevalence of AKI in this patient population remains high, with often unfavorable outcomes. This study was conducted in a multidisciplinary pediatric clinic in Uzbekistan and included 83 children diagnosed with nephrotic syndrome. Gender distribution showed a predominance of males (65.1%) compared to females (34.9%). The mean age was 7.2±5.6 years. AKI was diagnosed in 19 (22.9%) patients based on KDIGO criteria. The main risk factors for AKI included sepsis (26.3%), exposure to nephrotoxic medications (15.8%), acute post-streptococcal glomerulonephritis (15.8%), acute respiratory distress syndrome (10.5%), and renal parenchymal injury (10.5%). According to KDIGO classification, stage 1 was observed in 5.3% of patients, while stages 2 and 3 were found in 47.4% each. The mean hospital stay was 7.1±3.8 days, significantly longer in children with AKI. Mortality was 7.4%, with higher rates observed in cases of sepsis and drug-induced kidney injury. These findings demonstrate that AKI in children with nephrotic syndrome is associated with prolonged hospitalization, the need for intensive care, and an increased risk of adverse outcomes. Improving outcomes requires early diagnosis, active identification of risk factors, and preventive measures. The authors emphasize the necessity of multicenter studies in Uzbekistan to better define risk factors and optimize treatment strategies.
