ENDOSCOPIC METHODS FOR THE DIAGNOSIS AND TREATMENT OF CHOLEDOCHOLITHIASIS COMPLICATED BY OBSTRUCTIVE JAUNDICE

Resume

Study objective: To evaluate the effectiveness of a comprehensive approach to the diagnosis and treatment of choledocholithiasis complicated by obstructive jaundice. Materials and methods: The study included 128 patients with choledocholithiasis treated in the surgical department of Tashkent City Clinical Hospital No. 1 between 2018 and 2022. Patients were divided into two groups: the study group (n=62) – modern endoscopic methods (mechanical lithotripsy, biliary stenting, nasobiliary drainage) were used; the control group (n=66) – standard endoscopic treatment methods. All patients underwent ultrasound examination (US), esophagogastroduodenoscopy (EGDS), magnetic resonance cholangiopancreatography (MRCP), and endoscopic retrograde cholangiopancreatography (ERCP). Results: MRCP verified choledocholithiasis in 112 (76.6%) patients. Endoscopic papillosphincterotomy (EPS) was performed in 151 patients. In the study group, mechanical lithotripsy was used in 19.4% of cases, biliary stenting in 38.7% of patients, and nasobiliary drainage in 22.6%. Complication rates were: ERCP-induced pancreatitis in 1.6% and bleeding from the papillotomy site in 1.4%. High-tech endoscopic interventions were used significantly more frequently in the study group compared to the control group (p<0.0000001). Conclusions: The combined use of modern minimally invasive endoscopic technologies for choledocholithiasis complicated by obstructive jaundice is characterized by high efficacy and a low complication rate. MRCP combined with ERCP provides accurate diagnosis and the possibility of minimally invasive treatment.
