Post-stroke seizure syndrome represents a clinically significant complication of cerebrovascular diseases, substantially affecting prognosis and functional outcomes after stroke. The aim of this study was to perform a clinical and differential analysis of post-stroke seizure syndrome depending on stroke type, severity, and the timing of seizure onset. This retrospective and prospective study included patients with ischemic and hemorrhagic stroke complicated by seizure syndrome, as well as a control group of stroke patients without seizures. Comprehensive clinical assessment was performed using neurological examination and standardized scales, including the National Institutes of Health Stroke Scale (NIHSS), Barthel Index, and modified Rankin Scale. Seizure frequency, types, and clinical characteristics were analyzed in relation to stroke severity and seizure onset timing. The results demonstrated that post-stroke seizure syndrome following hemorrhagic stroke was associated with a more severe clinical course, higher seizure frequency, and a predominance of generalized seizure types compared to ischemic stroke. Late-onset seizures were characterized by significantly higher frequency and a tendency toward chronic epileptogenesis. A strong association was identified between increasing stroke severity according to the NIHSS and the transformation of seizures from focal forms to bilateral tonic–clonic and generalized seizures. The presence of seizure syndrome was associated with greater neurological deficits, increased functional dependence, and higher disability levels. In conclusion, stroke type and severity are key determinants of the clinical presentation and prognosis of post-stroke seizure syndrome. These findings highlight the importance of early risk stratification and the development of individualized therapeutic and rehabilitation strategies for patients with post-stroke seizures.
